Partnerships in dreams together
We provide a community based support service for people experiencing mental health issues on their way to recovery.
Care Coordinators work in partnership with you and DHB Clinical Teams (Intensive Community Treatment Team: your Nurse, Community Support Worker, Occupational Therapist and Psychiatrist).
WHAT DO WE OFFER
We can assist you to find housing in the community of your choice.
We can support you with your finances, daily living skills, health, education, employment, connecting you with family and friends, as well as your spiritual and cultural needs.
Resources in the community
As part of your journey to recovery and independence, we network with other community organisations which can assist you to reach your goals.
These include rental agencies and landlords, tertiary institutions or other training options, employment agencies, GP's, financial institutions, banks, legal advisors, gym, sport and recreational clubs and cultural groups.
HOW TO ACCESS THIS SERVICE
Anyone over the age of 18 who has a mental Illness and who is a client of the Counties Manukau District Health Board (CMDHB) Intensive Community Treatment Team (ICT).
Referrals are made via the clinical staff of the CMDHB Intensive Community Treatment Team.

Availability
Our team is available 24 hrs 7 days a week.
We are contactable after hours and weekends on an on call number to ensure that we can support you when you need us.
