Comprehensive Support

Affinity's Comprehensive Support Program (CSP) offers community based support to people who experience mental health issues on their way to recovery.

The CSP team is comprised of Mental Health Support Workers, supported by a Consultant Psychologist, Occupational Therapists and an Employment Consultant.

WHAT DO WE OFFER

  • We are able to regularly visit you at home, or where you choose, during the week, depending on your needs.
  • We will work with you to identify and build your strengths and explore your goals and dreams.
  • We will help address any difficulties you need to overcome along the way.
  • We will also work closely with your key worker and Doctor, as well as any other supports or whanau members you are close to, ensuring that we all work towards your goals.

HOW TO ACCESS THIS SERVICE

The service is provided to people who need ongoing intensive support with high support needs.

  • You can apply through your DHBKey Worker
  • You need to be a resident within the Auckland District Health Board or Waitemata District Health Board district(s).
  • Anyone over the age of 18 who has a mental health diagnosis, such as:
    • Schizophrenia;
    • Bipolar Disorder;
    • Depression; or
    • Anxiety disorders
  • If you have had one or more hospitalisation(s), respite or crisis contacts in the last year.
  • If an intensive support service would help you reach your goals.
  • If you are living in the community: independently, with your family, or flatting/lodging with others.
